November is National Healthy Skin Month!
It’s the first thing we see or touch, it covers us all from head to toe and shields us from the outside world – our SKIN.
So… with all that your skin does for you, what have you done for it lately?
The American Academy of Dermatology (AAD) implemented November as Healthy Skin Month in order to raise awareness about the importance of keeping skin fresh, hydrated and healthy all year round.
All month long, people are encouraged to learn about the functions of skin and how to keep it healthy and looking good.
Understanding how the skin functions starts with understanding the structure of the three layers of skin – the epidermis, dermis, and subcutaneous tissue.
The epidermis is the outer most layer of skin and contains five layers. The top layer of the epidermis, the stratum corneum, is made of dead, flat skin cells that shed about every two weeks.

The next layer is the dermis made up of Connective tissue that supports the epidermis, providing nutrients and protecting it.
The lower most layer is the subcutaneous tissue. The subcutaneous tissue is a layer of fat and connective tissue that houses larger blood vessels and nerves. This layer is important in the regulation of temperature of the skin itself and the body. The size of this layer varies throughout the body and from person to person.
Each of these layers plays an important role in the way our skin functions and looks. As a result, any breakdown in one of our skins layers causes various skin issues that we are all familiar with; be it acne, wrinkles and even good old aging.

CORE™ Technology: Understanding Penetration Depths of Different RF Modes
Reaction™’s CORE™ technology introduces a new dimension in RF aesthetic treatments that allows practitioners to independently control heating depth, but how and why is this important?
First though, what is Radio Frequency and why is it important to control it in the aesthetic world?
Radiofrequency (RF) energy is a form of electromagnetic energy. When applied to tissue, it produces electromagnetic fields, which results in the generation of heat. This heat effect has various aesthetic applications such as treatment of cellulite, tightening of lax skin, and for the reduction of thigh and buttocks circumference. Radiofrequency used for aesthetic purposes can be either monopolar (meaning a single electrode tip) or bipolar (two electrode tips). Since bipolar energy is more localized, less energy is required to achieve the same heating effect thus allowing safer treatments.
Radio frequencies’ aesthetic effects are based on a mild heating of the skin’s underlying network of collagen and elastin fibers which can lead to collagen shrinkage and thickening, improving skin’s firmness and elasticity. To this end, depth control of the heating effect is highly desired since different aesthetic imperfections originate from different skin layers and in different anatomical sites the thickness of skin layers varies significantly.
What is CORE™ technology?
Reaction™’s CORE™ technology incorporates unique variable frequency settings to precisely control RF penetration depth in underlying tissue. Reaction’s four different frequency modes can be independently chosen by the operator for any energy and probe setting to control the heating depth effect as required with each patient and each application. In addition, CORE™ technology incorporates tissue vacuum massage which assists in achieving even greater heat penetration which is important when treating the subcutaneous layer.
To prove that the Reaction™ does what Viora says it does, an in-vivo study was conducted using a thermal video camera to determine the tissue heating depth effects at the different RF frequencies available with CORE™ technology in the Reaction™ system. The Reaction™ system was applied on animal skin using protocols similar to those recommended for the different human aesthetic applications using Reaction’s three different RF applicators which are intended for large body areas (BC), small and sensitive body areas (FC) and smaller areas using RF energy only (ST).
Results from the study showed that heating depths for the different RF frequencies confirm the theory that RF penetration depth in tissue is an inverse function of frequency. For all applicators tested, heating depth observed was greatest for the lowest RF mode (0.8 MHz) applied and smallest for the highest frequency (2.45 MHz). As an example, for the ST applicator (no vacuum) heating depths measured were 7.2 mm, 5.1 mm & 3.9 mm corresponding to frequencies of 0.8 MHz, 1.7MHz and 2.45 MHz. A total heating depth range from 3.9 mm to 18.6 mm was measured for the treatment parameters used in the study on porcine skin.
To summarize, the unique variable frequency capability in Reaction™’s CORE™ technology, in addition to the ability to vary tissue heating effects by changing RF intensity level, vacuum intensity level and applicator configuration, allows utmost control when performing the full range of available RF aesthetic applications on different body areas and patient types.
Could Microdermabrasion Be Dangerous? Next Generation Treatments Alleviate Old Risks

From DIY scrubs to spas to doctor’s offices, microdermabrasion is the skin-beautifying treatment du jour.
Marketed as a non-surgical, safe and even relaxing way to get glowing skin, the traditional method most commonly blasts aluminum oxide crystals to sand the skin, and then suctions up the exfoliated dead cells and crystals with a vacuum.
But leading experts in the industry now warn of the potential dangers and side effects of the aluminum oxide crystals of yesterday, and say that a new generation of microdermabrasion technology has the potential to exfoliate the skin more safely and effectively.
“Microdermabrasion can be viewed as a medical treatment, even though it can be received in non-medical settings, and in some cases, post-treatment reactions are more intense. Some complications we read about were intense redness, infections, streaking from pressure on the probe, and post-inflammatory hyperpigmentation,” says P&G senior scientist Laura Goodman, who recently studied available methods of microdermabrasion on the market while creating a product for skincare brand DDF.
And as anyone who has had microdermabrasion knows, the aluminum oxide crystals often scatter haphazardly into hair, lashes and ears — which is not only bothersome, but possibly a health danger too.
“Crystals can cause irritation to the skin and eye exposure can cause corneal scratches and abrasions. Inhalation of the crystals and specifically the powder from the crystals can result in lung irritation and asthma exacerbation, which is more of a health concern for the professional administering the treatments,” says Dr. Matthew Schulman, assistant professor of plastic surgery at New York’s Mount Sinai School of Medicine.
There is also a safety concern for both moms-to-be and new moms.
“Aluminum oxide exposure has not been studied in child development and should therefore be avoided during pregnancy and breastfeeding,” advises Dr. Schulman.
Controversial links between aluminum in personal care products and increased rates of Alzheimer’s disease and breast and prostrate cancer are currently being studied, though the FDA says there is no conclusive evidence that would warrant regulation of products containing aluminum at the present time.
Two promising options have emerged in both the professional and at-home skincare market, that claim the same or better results without the baggage and possible side effects that come with aluminum oxide crystals.
For professionals, the Pristine Diamond Peel machine by Viora features a new generation diamond tip that exfoliates skin without the use of any crystals. The tips come in different levels of coarseness, so treatment can be customized to the patient’s skin type or to specific areas of the face.
StyleList recommends you chose a skilled and experienced professional if you go this route, as diamond tips can be abrasive if not used by experienced hands. Dr. Schulman uses the Pristine machine in his own office with much success.
“I truly believe that this crystal-free system is far superior to the traditional crystal systems. There is less irritation, it is safe during pregnancy and breastfeeding, and there is none of the annoying residual crystals in hair, nose and ears that can persist for days. Also, this system allows treatment of the sensitive areas around the eye without fear of eye injury,” says Dr. Schulman.

The energy control allows doctors to better target the treatment.
“So you are basically customizing the treatment for your particular body and your particular body part that you want to try to tighten.” says Dr. Sadick.
The energy burst triggers an initial tightening of existing collagen, but also sets off wound-healing.
Dr. Sadick says, “That wounding response actually causes your body to stimulate new cells, called fibroblasts, bring them to the area that’s heated, and those fibroblasts are the major cells that produce new collagen.”
Those fibroblasts work for months to prompt the skin tightening effect.
Dr. Sadick says, “You continue to tighten your collagen bundles, and that leads to improvement that continues for up to twelve months after a single treatment session.”
Results Phyliss hopes will help keep her at the head of the pack.
Treatments typically cost about $1,000 dollars each, though most doctors offer packages to bring down the cost. Depending on what you want to have done, four to eight 15-minute sessions are needed.

The difference between Skin Rejuvenation (IPL) and Skin Tightening (RF) treatment?
Skin Rejuvenation (SR)
SR treatments are indicated to improve the overall appearance and quality of the skin. SR treatments normally address the dermal and epidermal layers as a whole without specific target cells. SR treatments can be performed in several modalities; microdermabrasion (mechanical exfoliation), chemical peels, laser resurfacing and IPL.
IPL SR treatments
Target cells for IPL SR treatments are the skin cells in the dermis. The purpose of the IPL SR treatment is to increase the temperature in the dermal layer in order to create a controlled tissue trauma and encourage the body’s healing process by producing new skin cells.
Research has shown that exposing the skin cells to a temperature of 580C and above for one second leads to thermal damage, resulting in replacement of the cells with newer ones (Figure 1).
With the Trios™ IPL system, the skin temperature rises to ~650C for 25msec, and repetitive heating of the same tissue produces adaptive (cumulative) results leading to optimal rejuvenation without removal or burning of the epidermal layer (unlike peeling or exfoliating rejuvenation treatments which injure the epidermis layer).
As during the IPL SR treatment we are replacing skin cells, the amount and quality of elastic fibers or collagen are not increased or improved. The skin’s general appearance is improved but this outcome should be maintained frequently to continue replacing the dermal cells.

Skin Tightening (ST) with RF-based treatments
ST treatments differ from SR treatments in the purpose of the treatment and target cells. The purpose of ST treatments is to encourage the production of elastic fibers and connective tissue to improve skin elasticity.
RF based ST treatments
Target cells for the ST treatments are fibroblast cells located in the dermis. According to the stress model; to encourage fibroblast activity and gene expression of Collagen Type I and Type III, fibroblast cells must be heated to a temperature of 42°C for 4 minutes consecutively. The result of this process is popularly known as Collagen remodeling.
Using the Reaction™ ST applicator we are employing electromagnetic energy to produce a heat effect in the dermis where fibroblast cells are located. The multiple passes over the treatment area assure the cells remain at the effective temperature for 4 minutes. As a result, fibroblast cells increase their activity both to produce new connective tissue and replace existing or damaged tissue. The total amount of collagen in the skin is therefore increased and collagen quality is improved.
